What help or reassurance are you seeking 🙋‍♂️🙋‍♂️

The Maudsley Deprescribing Guidelines (published in 2024 by Dr. Mark Horowitz and Professor David Taylor) outlines essential instructions for safely stopping psychiatric medications like Trazodone. Importantly, the Maudsley guidelines focus entirely on psychotropic medications and do not provide advice on stopping Synthroid (levothyroxine). Synthroid is a thyroid hormone replacement, not a psychiatric drug. Stopping Synthroid can be highly dangerous and cause severe physical complications (such as severe fatigue, metabolic slowing, heart rate abnormalities, or a thyroid crisis). Never adjust or stop your Synthroid without explicit direction and laboratory monitoring from your doctor. What Maudsley 2024 Says About Coming Off Trazodone The core philosophy of the 2024 Maudsley guide is that standard medical tapers (like halving a dose and stopping) drop the dose too quickly at the exact point where the brain is most sensitive. • The Hyperbolic Tapering Principle: Maudsley establishes that as the dose gets smaller, the reductions must get even smaller. Even at lower dosages (often used off-label for sleep), your brain still develops physical dependence. • The Taper Speed: For a standard, moderate taper, Maudsley advises reducing the dose gradually by fractional percentages of the current dose over extended intervals. Rather than a straight linear drop, you reduce in a curve. • Micro-dosing at the End: The guide emphasizes that the final step before reaching complete discontinuation must be very small. To achieve these tiny, fractional doses, the guidelines recommend that doctors switch patients to liquid formulations or compounded versions. Symptoms to Expect (Antidepressant Withdrawal Syndrome) Withdrawal is a physical adaptation, not an addiction. When you reduce Trazodone, your brain takes time to down-regulate and adjust. Expect potential symptoms to manifest within a short timeframe following a reduction: Category Potential Symptoms Physical & Sensory Dizziness, headaches, nausea, sweating, tremors, and "brain zaps" (electric-shock sensations in the head or limbs) Sleep Disturbances Rebound insomnia, highly vivid dreams, and nightmares Mood & Psychological Heightened anxiety, irritability, crying spells, and panic attacks How to Cope and Manage the Discontinuation 1. Distinguish Withdrawal from Relapse: Maudsley 2024 emphasizes knowing the difference. Withdrawal symptoms appear very quickly (within days of a dose drop) and often include physical sensations like dizziness or zaps. A true relapse of depression or anxiety typically takes weeks or months to slowly develop. 2. Listen to Your Body (The Patient-Led Pace): Tapering should be flexible. If you make a reduction and feel severe withdrawal symptoms, hold your current dose for an extra period until your body stabilizes. Never speed up a taper to "catch up." 3. Collaborate with Your Doctor: Present the Maudsley Deprescribing Guidelines framework to your provider to request a liquid prescription of Trazodone so you can make accurate, small percentage drops. 4. Protect Your Sleep Hygiene: Because Trazodone is frequently prescribed to aid sleep, expect temporary sleep disruption. Keep your bedroom completely dark, avoid screens before bed, and keep a consistent wake-up time to anchor your circadian rhythm.
