Hi Clara , 20 years is a long time on it, so the main thing I'd say is don't let anyone swap you straight over. After that long your body's used to it, and coming off tramadol is famously rough (the usual opioid stuff plus anxiety and weird pins and needles). It has to be slow. Naproxen and nefopam aren't really replacements. They work on different pathways hut do have their own downsides. They are mainly symptomatic treatment, not for chronic. Good luck, you're asking the right things.

Hi @Clara! I'm sorry you've been struggling with back pain. Have you tried anything like acupuncture, massage, or stretching? Does using ice or heat help? Have you ever had an MRI of your spine/back? I did check about long-term effects of Tramadol: AI Overview Long-term use or misuse of tramadol changes brain chemistry, leading to physical dependence, cognitive decline, and lasting mental health challenges. National Institutes of Health (NIH) | (.gov) Dependence and Brain Rewiring Receptor adaptation: Tramadol binds to opioid receptors and affects neurotransmitters like serotonin and norepinephrine. Over time, the brain adjusts its chemistry to match the constant presence of the drug. Hanley Center Physical and psychological dependence: The brain relies on tramadol to feel normal, which triggers intense withdrawal symptoms and cravings if use stops abruptly. National Institutes of Health (NIH) | (.gov) Reward system changes: Prolonged exposure alters reward pathways, making it difficult to feel pleasure or motivation without the medication. Cognitive and Neurological Effects Cognitive impairment: Long-term use can reduce focus, slow reaction times, and damage short-term memory. Structural brain changes: Studies suggest that chronic exposure may cause oxidative stress and neuronal damage, particularly in memory centers like the hippocampus. National Institutes of Health (NIH) | (.gov) Seizure risk: Tramadol lowers the seizure threshold by affecting brain activity and inhibiting certain chemical signals, a risk that can persist or increase with continued use. Alta Mira Recovery Mental Health Impacts Mood disturbances: Extended use is linked to chronic anxiety, severe depression, and unpredictable mood swings. Psychiatric symptoms: High doses or long-term misuse can sometimes provoke confusion, paranoia, or hallucinations. Have you noticed any of these changes in yourself over the years? You can always research more holistic ways to handle your back pain. I'll be happy to provide further help if you need me to. If you desire to get off the Tramadol, as @Based said you would need to taper it instead of suddenly stopping it. I hope this helps! Catina 🌸

Thank you catina for all your information ... yes I paid for a scan as was getting no help from the doctors who just tell me to do Pilates ... my scan showed degenerative disc normal wear and tear I was told and abit of arthritis in facet joint .... I've tried most things ... currently trying acupuncture .. heat helps massively so just got a bob and brad back pad on her recommendations ... always trying different back programmes too .... Here in the uk they don't do lower preparations for tramodol withdrawal so just tell you to reduce one 50 mg pill one week two the next week and so on .... which is where I'm at .... Clara x

Glad you're trying lots of holistic treatments for your back pain. I too find heat incredibly helpful for pain. I hope your new pad is a success!

Yeah, they don't make it easy. You'd have to either shave down or crush your pills and then weigh them to taper. We have information on how to do this if you need it. It's under Tapering Protocols in the forums.

Well, "confusion" can happen as we age lol, but it an also happen due to long-term use of Tramadol. The jaw tightness and body shutting down definitely sounds withdrawal-related.
