I have kept a daily diary going back 150 days which I send to my psychiatrist every week.

Woke at 7:45am Symptoms of movement in my toes, calf muscles and some other areas came in n and I had no choice but to get up. I had diarrhoea straight away. Then burnt ing sensations in my abdomen and more movements in the areas described. It’s now 10:14am and these symptoms continue. Again I state that it is my personal wishes to stay on my present medication regime until these symptoms settle no matter how long it takes and then do a hyperbolic taper as now recognise in so many countries and Maudsley 2024!
Note;
The publication you are referring to is The Maudsley Deprescribing Guidelines: Antidepressants, Benzodiazepines, Gabapentinoids and Z-drugs (First Edition, 2024), co-authored by Dr. Mark Horowitz and Professor David Taylor. [1]
The core guidelines and rationale behind this specific instruction include:
* The "One-at-a-Time" Rule: When a patient is taking multiple psychotropic medications (polypharmacy), the general rule outlined by the authors is to prioritize and taper only one drug at a time.
* Isolating Withdrawal Symptoms: The primary medical reason for this is diagnostic clarity. If a person tries to reduce or stop two medications simultaneously and experiences a severe spike in anxiety, panic, insomnia, or physical distress, it becomes impossible to determine which drug's withdrawal is causing the symptoms. [1]
* Order of Prioritization: For patients taking both an antidepressant and a benzodiazepine, the manual generally suggests tapering the antidepressant first while keeping the benzodiazepine at a stable, continuous dose to help cushion or manage the potential withdrawal symptoms of the antidepressant. Once the first drug is completely stopped and the patient's central nervous system has stabilized, the tapering of the benzodiazepine can begin.
* Hyperbolic Tapering Method: For all included drug classes (including benzodiazepines), the manual outlines that stopping must be achieved via hyperbolic tapering—meaning reductions become progressively smaller as the dose gets lower, rather than dropping by a fixed milligram amount each time. [1, 2]
