Hi. I'm sunflower. I need help please. Tidal wave of severe menopause symptoms following abdominal surgery. Can't take HRT. Insomnia, night sweats, hot flushes, nausea, gastric irritation, intense anxiety and jittery feeling. Raised morning cortisol. Tinnitus. Cymbalta 30mg July 2024. for 1 week. Dose increased to 60mg for 2 weeks but caused urinary retention. Reduced back to 30mg. Horrendous symptoms persisted for 4 months then stabilised and felt ok. Stayed on 30mg until June 2025. Started 10% current dose reduction and held 2-3 weeks. Did really well with very occasional symptoms every now and then. January 2026 made huge mistake in my calculations. Went from 6.4mg to 5.6mg, held 2 weeks then went to 4.1mg. Onslaught of symptoms again. Updosed after 2 days to 4.8mg but stupidly, only held for 2 weeks and dropped to 4.4mg then 4.1mg. Knew I had messed up. Held dose for 7 weeks. No improvement. Updosed to 4.2mg. ( all this was bead counting). No stabilisation. Swapped to compounded pharmacy prepared duloxetine liquid. Held 4.2mg for another 3 weeks. Gastric irritation went insane. Back to beads. Still holding 4.2mg. it's been 5 weeks. I'm totally stuck and feeling horrendous. Don't know what to do. I understand the hyperbolic taper, the hyperbolic curve, the cliff edge and huge jumps in effort of neuro recalbration at the lower doses of duloxetine. I totally accept I messed up and tapered too fast. I am very upset I did. But now I'm stuck and need advice. Do I stay at 4.2mg to avoid yo-yo effect of going up again on my dose? Do I go back to 5mg and hold for months and then start again? Please help me

17-Aug-2026 (7.5mg -> 5mg)
9 months - actively tapering
- Procyclidine 5mg - 9 months - actively tapering
- Propranolol - 20mg - 1 year - holding
- Xanax - 0.25mg - 7 months - holding

Hi @Sunflower...welcome! I hear the frustration and desperation in your post. First, please don't beat yourself up too much about your calculation error. You have not done permanent damage or ruined the rest of your taper. It has happened to many of us at one time or another. Your nervous system is just trying to play catch-up with all the recent changes. What it needs right now is stability. If it were me, I would hold at the 4.2 mg to give your brain enough time to adapt. Five weeks may not be long enough. After you have stabilized, then you can proceed again at a rate you can handle. Since the bead counting is working better for you, I would stick with that from here on out and not try to switch to another formulation. Your body needs to keep everything stable and consistent right now. I hope this helps! I know how challenging this process can be, but we're here to help anytime you need to reach out. Catina 🩷
