Akathisia

Akathisia is one of the most distressing symptoms people can experience during psychiatric drug withdrawal or medication changes. It is often misunderstood and frequently mistaken for anxiety or agitation, but akathisia is a distinct neurological condition involving extreme inner restlessness. Understanding what akathisia is and how to cope with it can help reduce fear and provide practical strategies for relief. What Is Akathisia? Akathisia comes from a Greek word meaning “inability to sit still.” It is characterized by a powerful internal drive to move, often accompanied by intense psychological distress. People experiencing akathisia may feel: • An unbearable inner restlessness • A compulsion to move, pace, or shift positions constantly • Intense agitation or irritability • Inner torment or a feeling of wanting to “escape your own body” • Anxiety or panic that feels different from typical anxiety • Difficulty sitting, lying still, or relaxing Many people describe akathisia as one of the most uncomfortable sensations imaginable. It can occur when: • Starting a psychiatric medication • Increasing a dose • Reducing a dose • Stopping a medication It is especially associated with withdrawal from antidepressants, antipsychotics, and benzodiazepines. Why Akathisia Happens Psychiatric drugs alter several brain systems, particularly those involving dopamine, serotonin, and the nervous system’s stress response. During withdrawal, the brain must readjust after long-term receptor adaptations. This period of neurological instability can lead to symptoms like akathisia. Possible contributing factors include: • Dopamine receptor disruption • Nervous system hyper-activation • Rebound effects after medication reduction • Increased sensitivity in the central nervous system Withdrawal akathisia is not psychological and not a character flaw—it is a physiological nervous system reaction. What Akathisia Can Feel Like People in withdrawal communities often describe akathisia as: • “Like electricity running through my body” • “An unbearable urge to move” • “Feeling like I’m crawling out of my skin” • “A mix of terror and restlessness” • “Internal agitation that never shuts off” Understanding that this is a known withdrawal symptom can help reduce the fear that something is “going crazy” or permanent. Natural Strategies That May Help There is no instant cure for akathisia, but many people in withdrawal communities have found certain approaches helpful in calming the nervous system. Gentle Movement Because the body feels compelled to move, controlled movement can sometimes reduce the distress. Helpful activities may include: • Slow walking • Gentle stretching • Rocking in a chair • Pacing in a calm environment Trying to force the body to stay still can sometimes worsen the discomfort. Nervous System Calming Techniques Since akathisia involves an overactivated nervous system, calming practices may help reduce intensity. Examples include: • Slow diaphragmatic breathing • Listening to calming music • Guided relaxation or meditation • Gentle humming (which may stimulate the vagus nerve) • Warm baths or showers These techniques will not eliminate akathisia immediately but may help reduce the overall stress load on the nervous system. Reduce Stimulation During akathisia, the nervous system can be extremely sensitive. Helpful adjustments may include: • Limiting caffeine • Avoiding alcohol • Reducing sugar intake • Avoiding overstimulating environments • Taking breaks from screens and bright lights A quiet, low-stimulus environment can sometimes make symptoms more tolerable. Support and Reassurance Akathisia can feel incredibly isolating and frightening. Connecting with others who understand the experience can provide emotional relief. Many people find comfort in: • Online withdrawal support communities • Talking with trusted friends or family • Reminding themselves that the symptom is temporary Knowing others have experienced this and recovered can make a significant difference during difficult moments. A Note on Healing Akathisia can be one of the most challenging symptoms of withdrawal, but many people report that it gradually improves as the nervous system stabilizes. Recovery often happens slowly and in waves, but the brain and nervous system are capable of healing over time. Patience, nervous system support, and gentle self-care can help the body move through this process.
