
Pregabalin is a Schedule V gabapentinoid with a discontinuation syndrome that is more consistently reported, more rapid in onset, and more likely to include seizure risk than its structural cousin gabapentin. Prescribers increasingly encounter pregabalin dependence in patients treated long-term for neuropathic pain, fibromyalgia, or generalized anxiety disorder (an off-label indication in the US, approved in the EU), and abrupt discontinuation — whether due to nonadherence, hospitalization, or insurance lapse — carries meaningful clinical risk. This article reviews pregabalin's pharmacokinetics, the evidence base for its withdrawal syndrome, and a dose-dependent tapering protocol.
Pregabalin has a short elimination half-life of approximately 6.3 hours in patients with normal renal function, with steady state reached within 24–48 hours of a stable dosing regimen. It is not hepatically metabolized and has no active metabolites; more than 90% is excreted unchanged renally. This has two direct implications for tapering:
Available formulations are immediate-release capsules and oral solution (25 mg, 50 mg, 75 mg, 100 mg, 150 mg, 200 mg, 225 mg, 300 mg) and an extended-release tablet (Lyrica CR, 82.5 mg and 165 mg), approved only for neuropathic pain associated with diabetic peripheral neuropathy and postherpetic neuralgia, dosed once daily with a meal. The IR-to-CR conversion is not 1:1 by tablet strength; CR labeling specifies a separate titration schedule, and tapering an established CR regimen should use CR strengths rather than converting to IR mid-taper unless clinically necessary.
Pregabalin discontinuation symptoms were documented in the original pivotal trials and are now well characterized in postmarketing pharmacovigilance and observational cohorts.
Symptom profile. The most frequently reported symptoms across studies are insomnia, nausea, headache, diarrhea, anxiety, hyperhidrosis, and — less commonly but of highest clinical concern — seizures, including in patients with no prior seizure history. A 2013 systematic review by Guglielmo et al. and the FDA-approved labeling both note that discontinuation symptoms occurred in up to 12–16% of pregabalin-treated patients in placebo-controlled trials who received the drug for as little as several weeks, with onset generally within the first 24–48 hours after the last dose and duration typically 1–2 weeks, though a persistent minority report protracted symptoms lasting months.
Dose-response relationship. Discontinuation symptom incidence and severity scale with total daily dose and, to a lesser extent, treatment duration. Patients on doses at or above 300 mg/day and those treated for longer than 12 months show substantially higher rates of moderate-to-severe discontinuation symptoms in cohort data reviewed by the Maudsley Deprescribing Guidelines (Horowitz & Taylor, 2024), which extend hyperbolic tapering principles — originally developed for antidepressants and benzodiazepines — to gabapentinoids on the basis of shared GABAergic and calcium channel modulation mechanisms.
Seizure risk specifically. Pregabalin binds the alpha-2-delta subunit of voltage-gated calcium channels, reducing excitatory neurotransmitter release. Abrupt cessation, particularly at higher doses, can produce a rebound hyperexcitability state. The FDA label for Lyrica carries a specific discontinuation warning: taper over a minimum of 1 week when discontinuing, regardless of indication, due to reported cases of seizure — including in patients without epilepsy — following abrupt withdrawal. This is a labeled, not merely observational, risk.
Differentiating withdrawal from relapse. In patients treated for neuropathic pain or fibromyalgia, distinguishing withdrawal-related paresthesia and myalgia from disease recurrence requires attention to timing: withdrawal symptoms cluster within days of a dose reduction and typically resolve or plateau within 1–2 weeks, whereas pain relapse tends to build more gradually and does not remit spontaneously. In GAD, rebound anxiety from discontinuation is often difficult to distinguish from underlying illness recurrence without a prior treatment history for reference; a slower taper reduces this diagnostic ambiguity by lowering peak symptom intensity.
There is no FDA-mandated taper rate beyond the minimum 1-week discontinuation window; the following schedule reflects a synthesis of the Maudsley Deprescribing Guidelines' hyperbolic approach and typical clinical practice for patients on stable long-term therapy. It should be adjusted for renal function, dose, treatment duration, and individual tolerance.

For patients with a history of significant discontinuation symptoms, prior seizure activity, or doses above 450 mg/day, a slower protocol — reducing by 10% of the current dose every 2–4 weeks rather than fixed percentage cuts — produces smaller absolute reductions as the dose falls, consistent with hyperbolic tapering logic and receptor occupancy curves that flatten at lower doses. This requires compounding or oral solution for increments below the smallest capsule strength (25 mg).
Minimum taper duration for acute/inpatient settings: at least 1 week, per FDA labeling, split into at least two step-downs rather than a single discontinuation.
Minimum taper duration for long-term outpatient use (>3 months, especially >300 mg/day): several weeks to a few months; patients on doses above 450 mg/day for over a year may require 3–6 months for a low-symptom taper.
Dosing should remain divided (BID or TID) throughout the taper given the short half-life; consolidating to once-daily dosing before the total daily dose is low risks larger peak-trough swings and increased breakthrough symptoms.
Although both are gabapentinoids and share a discontinuation symptom profile, pregabalin's linear, saturable-transporter-independent absorption (versus gabapentin's dose-dependent, saturable absorption via the LAT1 transporter) means pregabalin's exposure scales predictably with dose, while gabapentin's does not. This makes percentage-based tapering more pharmacokinetically valid for pregabalin than for gabapentin, where equivalent percentage dose reductions at high doses may produce disproportionately large drops in plasma exposure due to absorption saturation. Do not assume a gabapentin taper protocol transfers directly to pregabalin dosing logic.
